READ MORESupporting Every Phase of Alzheimer’s

Alzheimer’s and dementia are well-known diseases that affect people’s lives as they age. Supporting people with memory inhibiting diseases is something we do every day. We see the challenges that Alzheimer’s causes on every front, and are here to provide helpful resources and offer respite and encouragement. Since Alzheimer’s and dementia progress gradually, we get to enter people’s lives at different stages of these conditions. Here are some of the ways we can help.

Preparation Period

With increased awareness and recent medical advances, it is possible to detect Alzheimer’s disease early. This gives people with Alzheimer’s the ability to make advance directives. We can help these people assess their needs and plan for the future.

After Accidents

Sometimes people are officially diagnosed with Alzheimer’s disease after it has progressed. Alzheimer’s often leads to falls, injuries, wandering and unsafe living conditions. It is not uncommon for healthcare professionals and family members to be unaware that a patient or loved one has Alzheimer’s until after these events occur.

This can be emotionally difficult for the patient and their family. Many people don’t know where to turn or who to call for guidance and comfort. At the same time, these people know that they need help as soon as possible. Many case managers and social workers can help families determine the best care for their situation. SYNERGY HomeCare can also help these families make customized care plans.

From Far Away

Many adult children live in different cities and even different states from their senior loved ones. Even though these family members communicate regularly over the phone or by video chat, it is during visits (especially during the holidays) that their eyes are opened to their loved one’s condition. We coordinate care with families and communicate with them about their loved one’s status.

In Assisted Living

There are many levels of assisted living. Oftentimes, people with Alzheimer’s can stay in the more independent levels with the help of a caregiver. SYNERGY HomeCare works with assisted living facilities and independent living communities to provide the best care.

At the End of the Rope

We highly regard the many family members who step up to the plate when their love one is diagnosed with Alzheimer’s disease or dementia. As the disease progresses and more and more is required of these family caregivers, it is easy for them to become burned out. It can even put their jobs and other responsibilities on hold. We receive calls from many people at the end of their rope, and are glad to provide care for their loved ones and give them the peace of mind and respite they need.

SYNERGY HomeCare is proud to be the premier provider of home care to people with Alzheimer’s disease and dementia. Please contact your local SYNERGY HomeCare agency for more information about our Alzheimer’s care programs.
