DISABILITY CARE
At SYNERGY HomeCare, we provide the personal assistance and care you need to complete daily tasks while living safely and comfortably at home. Disability assistance can come in the form of in-home care support services such as meal preparation, transportation and housekeeping, and/or assistance with hygiene and personal care. Our goal is for you to be confident in your abilities and comfortable with our compassionate care so that you can live your best life, wherever you call home.
CANCER CARE
Going through the many steps and stages of a cancer diagnosis, treatment, recovery or relapse can be overwhelming. You don’t have to face it alone. We are dedicated to helping you navigate the challenges and to help ease the added burdens that cancer can bring into one’s life. Our goal is to provide you the support you need to experience the least stressful and most comfortable healing and recovery process at home.
ALZHEIMER'S AND MEMORY CARE

PARKINSON’S CARE
We recognize that each person and family living with Parkinson’s disease is unique, and that a complex, progressive illness such as Parkinson’s requires an interdisciplinary approach and ongoing communication between care providers, medical professionals and family members. From moderate personal assistance to full-time care, we can support your in-home Parkinson’s care plan to help you meet your particular goals and needs, including help with healthy meal planning, transportation to and from appointments, and medication reminders.
END-OF-LIFE COMFORT CARE
We believe that end of life comfort care is a gift of maximizing precious time. SYNERGY HomeCare professional caregivers work closely with your hospice program to provide an extra level of in-home care, allowing you to be present with your loved one not as a family caregiver, but as just yourself. We can assist your loved one with personal assistance, overnight care or 24/7 care. We are committed to providing the most dignified and respectful end of life comfort care and support for your loved one, your family, and anyone else sharing in grief.
NEUROMUSCULAR CARE
There is a continuum of needs for individuals living with muscular dystrophy and neuromuscular disease, requiring different levels of care and support. We recognize that the first step in determining what type of care is most appropriate for your loved one is to first meet for a complimentary in-home assessment. During this visit, we want to learn more about how daily activities are impacted by the symptoms they are experiencing, so that together we can develop a customized plan of professional in-home care and support.
STROKE RECOVERY CARE
Dealing with the physical, emotional and behavioral changes after someone you love has survived a stroke can be difficult for each of you. This uncertainty can leave your family wondering how to cope with the immediate challenges of everyday life post stroke and facing long-term decisions about whether your loved one can remain comfortably at home. That’s where SYNERGY HomeCare comes in. Our stroke recovery care plans are customized to address a wide variety of services, ranging from hourly assistance to around-the-clock care, all by trained care professionals who work with you to create a plan that best suits your family’s needs.
POST-SURGICAL RECOVERY CARE
Whether planned or unexpected, surgery means that your body has gone through an experience that requires time to heal. SYNERGY HomeCare can be an essential part of your in-home recovery care team, smoothing the transition from operating room to living room. Life and the needs of a household don’t always wait, and if you “over do it,” your recovery from surgery could take longer than expected. Home care can be a great prescription when recovering from surgery, as it allows you to concentrate on regaining your health while a caregiver keeps up with light housekeeping, errands and transportation. A caregiver can also help in the surgery recovery process by creating healthy meals, timely medication reminders and setting up follow up health appointments.
DIABETES CARE
Living with diabetes can be challenging. From establishing a healthy lifestyle to managing blood sugar and insulin levels, diabetes can require supplemental care outside regular doctor’s visits or after a hospitalization for an acute episode. We recognize that managing your diabetes takes planning and support. The best diabetes care plan includes healthy nutrition, maintaining an active lifestyle, being prepared and avoiding additional medical complications. Our supportive caregivers can help with the planning and preparation of nutritious meals, medication reminders, and testing reminders, as well as to encourage regular exercise.
ARTHRITIS CARE
Gradually increasing joint pain and stiffness are the most frequent symptoms of arthritis, and can make the routines of daily living more difficult for your loved one. Whenever activities like eating, grooming, and housekeeping are hampered by stiffness and pain, the result can be long-term concerns with nutrition, personal hygiene and safety. The assistance of a skilled SYNERGY HomeCare caregiver can help your loved one living with arthritis maintain a healthy lifestyle in the comfort of home.
NURTURING CARE FOR NEW OR EXPECTANT MOMS
New or expectant moms can benefit from the in-home support of a trained SYNERGY HomeCare caregiver. We help expectant moms with laundry, light housekeeping and errands when rest is key. Our meal preparation can be tailored to issues of ongoing morning sickness or all-day nausea, gestational diabetes or high blood pressure. We can even assist with child care for your other children. Once home with your baby, our caregivers are available 24/7 to provide the relief you need to get a few hours of much-needed rest or even a full night’s sleep.
